not cleat related and i'm sure you've been to docs, but have you tried a patellar strap? I was having unknown knee issues that hurt worse when I went easy. I went to a doc who was team doc for 88 Olympic luge team, and he recommended stretching and this strap. It was amazing--my knee was better in like three days. I have to ice the $%^&amp;#38; out of it after a long hard ride, but its 100% better.

I'm sure you have a crack team of experts helping you, but consider re-checking your cleats.

I've had knee problems for the past two months, got professionally fitted, saw a physical therapist and everything.  I wasn't seeing any improvement so I finally said 'fuck it', I'm going to fix this.  Adjusted my cleats to rotate my heel in a bit further to match my neutral position while walking, and the pain/inflammation was 90% gone, just like that.  My muscles are still adjusting to the new position, but now I can feel that I'm getting better.

Just a thought - recheck everything you thought was set...it could simply be mechanical...</description>
